SOME IMPORTANT STRIKERS
- Metformin is the best medicine to start with, given to all except those with renal damage. It is not only an anti diabetic, also used for treating polycystic ovarian disease as it helps in weight reduction too.


- Pain killers used frequently for aches and pains can lead to kidney damage, specially earlier in a diabetic whose kidney is already weakened by the disease. Hence only paracetamol is advisable for pain.


- Insulin should be used in alternating sites of the 8 sites which can be used. Always self injection is advisable. The fastest absorption would be in the abdomen. It is commonly given subcutaneously. It is stored commonly in the fridge door. Some types are now available in pen forms both disposable and non-disposable.

- Insulin is the cheapest and the best anti diabetic medication with no chemicals causing side effect since it is a natural product. So do not hesitate to prefer and use insulin over tablets.

- Insulin secretion in normal individuals is in 2 forms, one is basal secretion, occurs all the time and another is burst secretion after food intake which subsides after a while.

- Fasting glucose level is more dangerous than post prandial. So we have to make sure it is in control. If fasting is high despite taking tablets, it means that basal insulin production has completely stopped in that individual due to complete destruction of beta cells of pancreas.

- Insulin dosage should be titrated according to the hypoglycemic episodes and hyperglycemia. Morning insulin dose can be increased substantially than the night dose due to the risk of Somogyi effect.

- Pioglitazone which was a good anti diabetic medication is about to be withdrawn from the market due to the risk of causing bladder cancer. Pioz can cause weight gain of about 12 kgs. If this happens, the drug should be immediately stopped. Rosiglitazone is already banned.

- Neuropill induces sleep while on it.

- Zomelis controls appetite thereby aiding a good diet control.

- Rabonik should be taken before food, so that it can reduce acid secretion.

- Just one banana can shatter the blood glucose control and accelerate it for a week. Avoid items like bread, cake, coke, fruits like banana, mango, jack fruit.

- Even passive smoking is dangerous for a diabetic.

- Alcohol and smoking can accelerate heart disease in a diabetic.

- Anemia can show falsely high HbA1c levels.

- Fasting and feasting both are dangerous in Diabetes.

- Food intake should be once in 3 hrs to prevent hypogylcemia when on medication.

- Fruits should be taken only twice a week. Preferably only apple, guava, orange, pomegranate and papaya. Only in cut pieces and not as juice since it deprives us of the fibre content.
If fruits taken more frequently than advised, then the fructose in fruits can get converted into more harmful glucose and add to the blood glucose level.

- Ringer lactate avoided in Diabetics, can use N/S and ISOLYTE-M in alternation.

- Anti-diabetic tablets are avoided during dialysis.
